Audit Trails Must Explain What Happened and Who Changed It

When someone asks what happened, the system should answer, not someone's memory. Nexum logs who did what, when and why.

When someone asks what happened, the system should answer, not someone's memory.

Summary

Audit trails answer questions from clients, consumers, regulators and managers.

They must capture actor, action, object, time, before and after, reason and source.

Nexum logs contacts, disputes, approvals, overrides, files and sign-ins where people can find them.

Key takeaways

Confirm what your system logs and what it doesn't.

Make history visible on the account.

Why audit trails matter

Who changed this balance? Who called, and when? Who approved this settlement? Who downloaded this file? The question always comes.

What a good trail records

  • Actor: which person or system did it.
  • Action: what was done (created, changed, deleted, approved, sent, downloaded).
  • Object: which account, payment, file or setting.
  • Time: when, with time zone.
  • Before and after: the prior and resulting state for changes.
  • Reason: for overrides and sensitive actions, why.
  • Source: manual, system, integration or AI-assisted.

What should be logged

  • Contacts on every channel.
  • Status changes.
  • Balance and payment changes.
  • Disputes and their stages.
  • Approvals and rejections.
  • Overrides of automated decisions.
  • File access, downloads and shares.
  • Sign-ins and permission changes.
  • Settings changes.

Make it usable

An audit trail that exists only in a database nobody can query isn't much help. The most useful trails are visible where people need them: on the account, on the file, on the user.

Protect it

Users shouldn't be able to edit or delete audit history. Access to audit logs should itself be controlled.

Test it

Pick a sample account and try to answer: every contact in the last 90 days, every status change and who made it, every payment and reversal, every approval. If you can't, find the gaps.

How Nexum records history

  • Comm Log: every contact with time, channel, direction, result, who, phone, call length, recording and source, plus system entries for status changes and tasks.
  • Disputes: filing and resolution dates.
  • BackOfficeHub: AI brief, decision, rejection reason and failed actions.
  • DebtorIndex: overrides with reason and the user's name in a decision log.
  • File Sharing: every upload, download, delete, link and share.
  • User Management: sign-in history.
  • e-OSCAR Agent: live activity log.

The bottom line
